Paid ads bring the first enquiries within three to seven days. Google Business Profile and Maps move over two to four weeks. Your own site's SEO takes three to six months. Reputation takes closer to a year. Judge nothing before day thirty, and judge the channel, not the month.

Ads move in days, everything else moves in months

Week one produces almost nothing you should judge. It is a setup week: installing tracking, writing the WhatsApp reply script, cleaning up the Google Business Profile. Paid ads on Instagram and Snapchat usually start returning enquiries within three to seven days of going live, and that is the fastest lever a clinic in Kuwait has. Do not judge cost per enquiry before day ten, because the platforms are still learning who to show you to. If someone promised you booked appointments in week one, they promised something the funnel cannot deliver yet.

Month one is where the honest picture appears. You should have a reasonably stable cost per enquiry, a first read on which treatment gets the clicks, and the beginning of a review flow if you started asking for reviews. Most clinics we work with land between 2 and 8 KD per WhatsApp enquiry in month one, and that range is wide because a routine dental check-up and a laser package are not the same purchase. The first patients rarely come from the campaign. They come from someone who already drives past your building in Salmiya, someone who searched your name after a friend mentioned it, or someone who saw four recent reviews instead of none. Proximity and reputation do more work in month one than any creative you run. That is why the cheapest thing you can do in the first thirty days is ask every satisfied patient for a Google review at the reception desk. It costs nothing, and it moves Maps ranking, which is what the person searching from 800 metres away actually sees.

Month three is Maps and repeat patients, month six is search

By month three, the Google Business Profile work should show. Clinics that post weekly, answer every review, keep photos current and hold a steady rating usually move into the local three-pack for their own neighbourhood searches, things like dentist Hawally or a dermatology clinic in Salmiya, somewhere between month two and month four. Organic SEO on your own site is slower. Expect three to six months before treatment pages rank for anything competitive, and closer to nine for the big generic terms. Anyone quoting SEO results in six weeks in Kuwait is selling you Maps and calling it SEO.

Month three is also the first month you can read retention. Look at how many month-one patients came back or referred someone. That single number predicts month twelve better than any ad metric does. Clinics with a retail line, skincare, aftercare products, supplements, see it earliest, because a patient who reorders is a patient who trusts you. If you are selling those products through WhatsApp screenshots and manual KNET links, move them onto a proper store so reorders happen without a staff member typing out a payment link every time.

Month six is where the compounding shows up. Reviews are in the hundreds rather than the dozens, people search your clinic name directly instead of your category, and cost per enquiry usually falls 20 to 40 percent against month one because the audience already recognises you. The budget stays the same and the output rises. This is also the point where you can cut the worst channel without fear, because you have six months of data instead of six weeks of noise. Plan the year around Ramadan and the July to August travel exodus, both of which flatten volume no matter how good the marketing is.

Enquiries without appointments is a front-desk problem, not an ads problem

If enquiries arrive and appointments do not, stop changing the campaign. The marketing already did its job. Measure two things instead: how many minutes pass before someone replies on WhatsApp, and how many enquiries get a specific time offered rather than a greeting and a question mark. Replying inside five minutes roughly doubles the booking rate against a reply two hours later. Most clinics in Kuwait lose their enquiries between 8 pm and midnight, which is exactly when people message and exactly when nobody is watching the phone.

The second cause is price and clarity. People ask, get no number back, and go quiet. You do not have to publish a price list, but the reply should give a range, say what the first visit includes, and offer the next two available slots.
