Why a bare number loses
It gives the patient exactly what they need to compare you against a clinic that will quote lower by excluding things you include. It also ends the conversation — there is nothing to reply to — so they leave and message the next clinic on their list.
Refusing is worse. "Please call us for pricing" is read as evasion, and the patient assumes you are expensive and hiding it. Somewhere between those two is the reply that works.

The script
"For [procedure] it starts from [X] KD. That includes the consultation, [what else], and [number] follow-ups. What takes it higher is [the one or two real factors]. Dr [name] has [day] at [time] or [day] at [time] — shall I hold one for you?"
Four sentences, sent as one message, in the language they wrote in. Saved as a quick reply so every member of staff sends the same thing, because the variation between team members is where most clinics quietly lose enquiries.
When they say it's too expensive
Do not discount, and do not defend. Ask what they were expecting, and then either explain what your number includes that a lower one does not, or offer the genuinely smaller option if one exists — a staged plan, a simpler alternative, or an assessment first.
A patient who books after being talked down on price becomes a patient who negotiates everything. A patient who books because they understood what they were buying does not.
